Home Health Services in Wyoming provide short-term, medically necessary clinical care to individuals recovering from illness, injury, surgery, or managing chronic conditions. Services are delivered by licensed professionals—such as nurses and therapists—in the participant’s residence, and are intended to prevent hospitalization or institutionalization. These services are covered under Wyoming Medicaid’s State Plan and may also be authorized through HCBS Waivers, including the Community Choices Waiver (CCW) and certain pediatric programs like EPSDT. All services must be ordered by a physician and documented in a Plan of Care (POC).

2. HOME HEALTH SERVICES OVERVIEW
Home health services focus on clinical interventions provided in the home to treat, stabilize, or manage a participant’s health condition.
Covered home health services may include:

Physical therapy, occupational therapy, or speech therapy
Medical social work
Home health aide support (e.g., bathing, basic care under supervision)

3. LICENSING & PROVIDER APPROVAL REQUIREMENTS

4. WYOMING PROVIDER ENROLLMENT PROCESS
