Residential Habilitation Services in Wyoming provide 24-hour support and supervision in structured home or group living environments for individuals with intellectual or developmental disabilities (I/DD). These services focus on helping individuals build daily living skills, improve personal autonomy, and engage meaningfully in community life while receiving the safety and consistency of residential support. Residential habilitation is a core service under Wyoming’s Comprehensive Waiver administered by the Wyoming Department of Health’s Developmental Disabilities Division (DDD). Services are highly individualized and delivered in accordance with a person-centered Individualized Plan of Care (IPC).

1. GOVERNING AGENCIES

Agency: Wyoming Department of Health (WDH) – Developmental Disabilities Division (DDD)

Role: Establishes standards, service definitions, provider enrollment requirements, and ongoing monitoring for Residential Habilitation.

Agency: Medicaid Provider Services – Division of Healthcare Financing

Role: Manages billing, claims processing, and reimbursement for waiver services.

Agency: Case Management Agencies

Role: Develop and monitor the Individualized Plan of Care (IPC), coordinate service authorizations, and track outcomes.

 

2. RESIDENTIAL HABILITATION SERVICES OVERVIEW

Residential habilitation offers 24/7 support in certified settings designed to enhance independence while ensuring health, safety, and daily structure.

Covered services may include:

  • Support with activities of daily living (ADLs) such as bathing, dressing, toileting, and mobility

  • Meal planning and preparation

  • Medication assistance and health monitoring (under nurse delegation if needed)

  • Skill-building in budgeting, hygiene, home care, and communication

  • Community participation and recreation

  • Behavioral support (as needed and outlined in the care plan)

  • Overnight supervision and emergency response readiness

  • Assistance with developing routines and autonomy

Services must be tailored to each individual’s goals and outlined in the IPC with specific outcomes and documentation requirements.

 

3. LICENSING & PROVIDER APPROVAL REQUIREMENTS

Prerequisites:

  • Register your business with the Wyoming Secretary of State

  • Obtain a Federal EIN and Type 2 NPI

  • Apply to become a DDD-certified Residential Habilitation provider

  • Complete the DDD provider certification process, including pre-service training

  • Develop a Residential Habilitation Policy & Procedure Manual per WDH guidelines

  • Ensure that all staff pass background checks and are trained in required health and safety protocols

  • Maintain residential safety standards, including emergency evacuation and medication storage procedures

  • Secure liability and property insurance

 

4. WYOMING PROVIDER ENROLLMENT PROCESS

Step 1: Register your business and obtain an EIN/NPI

Step 2: Apply for DDD provider certification as a Residential Habilitation provider

Step 3: Complete the required pre-service orientation and submit your P&P Manual

Step 4: Arrange for home inspection and compliance review

Step 5: Receive Medicaid enrollment credentials and billing ID

Step 6: Accept referrals from case managers and begin services upon IPC approval

 

5. REQUIRED DOCUMENTATION

  • DDD Provider Certification documents

  • Residential Habilitation Services Policy & Procedure Manual, including:

    • Admission, transition, and discharge protocols

    • Daily schedule and service delivery routines

    • Medication assistance and health tracking procedures

    • Safety drills, emergency planning, and staff communication tools

    • Behavior intervention and de-escalation practices

    • Staff supervision logs and training records

    • IPC alignment with daily progress notes and monthly summaries

  • Facility safety plans, cleaning checklists, and incident reports

  • House rules and consent documentation

 

6. STAFFING REQUIREMENTS

Role: Direct Support Professional (DSP)

Requirements:

  • Minimum age 18

  • Background check and abuse registry clearance

  • CPR/First Aid certification

  • Completed pre-service training through DDD

  • Ongoing in-service training (including fire safety, rights, and HIPAA)

Role: Program Manager / House Manager (recommended)

Requirements:

  • Supervises staff schedules, training, and client outcomes

  • Ensures compliance with care plans and agency procedures

All staff must complete:

  • Abuse and neglect prevention

  • Emergency preparedness and behavior intervention

  • Documentation and confidentiality protocols

  • Person-centered practices and disability rights

 

7. MEDICAID PROGRAMS THAT COVER RESIDENTIAL HABILITATION SERVICES

  • Comprehensive Waiver: Covers 24/7 residential habilitation services for individuals with I/DD who meet level-of-care criteria. Supports may be provided in provider-owned or -operated homes with 1–3 residents.

  • Supports Waiver: Does not cover 24/7 residential habilitation. Individuals needing full-time care must be served under the Comprehensive Waiver.

 

Note: Residential Habilitation is subject to service limits, staffing ratios, and WDH regulations. Services must align with the IPC and include documented outcomes and participant feedback.
