FOSTERING PARTICIPATION, SOCIAL CONNECTIONS, AND INCLUSIVE OPPORTUNITIES FOR INDIVIDUALS WITH DISABILITIES

Community Integration Services in Wyoming support individuals with disabilities in building meaningful relationships, participating in public life, and engaging with local resources. These services are designed to help individuals develop social skills, increase independence, and access recreational, educational, and cultural experiences outside the home. Community integration is a core component of Wyoming’s Comprehensive and Supports Waivers, both of which are administered by the Developmental Disabilities Division (DDD). Services must be individualized and outlined in each participant’s Individualized Plan of Care (IPC), with a focus on person-centered goals and community involvement.

 

1. GOVERNING AGENCIES

Agency: Wyoming Department of Health – Developmental Disabilities Division (DDD)

Role: Sets program standards, approves providers, and monitors compliance with waiver requirements.

Agency: Medicaid Provider Services – Division of Healthcare Financing

Role: Oversees provider enrollment, billing, and reimbursement for HCBS services.

Agency: Case Management Agencies

Role: Coordinate participant goals, develop the IPC, and authorize community integration supports.

 

2. COMMUNITY INTEGRATION SERVICES OVERVIEW

These services promote full inclusion in the community by supporting individuals as they engage in social, recreational, volunteer, and educational activities that match their interests and abilities.

Covered activities may include:

  • Attending community events, art shows, or festivals

  • Volunteering at a local nonprofit or place of worship

  • Visiting museums, libraries, or cultural centers

  • Participating in fitness or wellness classes

  • Attending continuing education or community college courses

  • Social skill development in public settings (e.g., ordering food, bus riding)

  • Recreational outings with peers and community members

  • Navigating transportation systems and building independence

  • Developing routines to access community resources

Activities must be goal-directed, inclusive, and appropriate to the individual’s abilities. Integration services are not intended to be repetitive or isolating and must offer exposure to typical life in the community.

 

3. LICENSING & PROVIDER APPROVAL REQUIREMENTS

Prerequisites:

  • Register your organization with the Wyoming Secretary of State

  • Obtain a Federal EIN and Type 2 NPI

  • Enroll with the Wyoming Medicaid system under HCBS waivers

  • Submit an application for certification as a Community Integration provider with DDD

  • Develop a Community Integration Services Policy & Procedure Manual

  • Hire staff trained in disability services, safety, and community support

  • Carry liability insurance and complete staff background checks

Licensure as a health facility is not required, but DDD provider certification and Medicaid enrollment are essential.

 

4. WYOMING PROVIDER ENROLLMENT PROCESS

Step 1: Register your business and obtain EIN/NPI

Step 2: Complete DDD provider application for community services

Step 3: Develop and submit P&P Manual with safety, supervision, and activity protocols

Step 4: Enroll with Medicaid via the Provider Portal

Step 5: Complete orientation or training as required

Step 6: Begin offering services upon IPC authorization

 

5. REQUIRED DOCUMENTATION

  • Medicaid provider enrollment documentation

  • DDD certification paperwork

  • Community Integration Services Policy & Procedure Manual, including:

    • Individual activity planning and tracking

    • Community safety, emergency, and supervision protocols

    • Transportation procedures and permission documentation

    • Staff-to-participant ratio standards

    • Risk management and incident response protocols

    • HIPAA compliance and confidentiality training

    • Participant rights and informed consent forms

    • Monthly progress reports and IPC goal tracking forms

  • Staff training logs, background checks, and supervision documentation

 

6. STAFFING REQUIREMENTS

Role: Community Support Specialist / Direct Support Professional (DSP)

Requirements:

  • Minimum age 18

  • High school diploma or GED

  • Valid driver’s license (if transporting participants)

  • Background check and abuse registry clearance

  • CPR/First Aid and community safety training

Preferred Qualifications:

  • Experience in recreation, disability services, or social work

  • Strong interpersonal skills and ability to promote social interaction

All staff must complete:

  • HIPAA and confidentiality training

  • Abuse and neglect prevention

  • Person-centered support and documentation

  • Emergency preparedness and community safety procedures

 

7. MEDICAID PROGRAMS THAT COVER COMMUNITY INTEGRATION SERVICES

  • Comprehensive Waiver (I/DD): Includes extensive community integration supports for individuals with high needs

  • Supports Waiver (I/DD): Offers community integration within budget limits and based on the individual’s goals

  • Children’s Services (under Waiver or EPSDT): Community participation and outings may be supported when tied to functional development or social skill-building

 

Note: Activities must not be duplicative of other services (e.g., day habilitation), and must be documented with a clear purpose and connection to the IPC goals.
