2. ADULT HEALTH SERVICES OVERVIEW

Approved providers may deliver:

  • Health Monitoring and Assessment: Regular checks of vital signs, chronic condition management, and wellness evaluations.

  • Obtain EIN from the IRS and NPI (Type 2).

  • Enroll as a Medicaid Waiver Adult Health provider via the New Hampshire Medicaid Provider Enrollment Portal.

  • Obtain licensure from DHHS as a healthcare service provider if applicable.

  • Maintain general liability and professional liability insurance.

  • Develop comprehensive policies for health monitoring, medication management, and wellness support.

4. NEW HAMPSHIRE PROVIDER ENROLLMENT PROCESS

  • Complete the Provider Enrollment Application through the New Hampshire Medicaid Provider Enrollment Portal for Adult Health Services under HCBS waivers.

Application and Documentation Submission:

  • Submit Articles of Incorporation, proof of EIN/NPI, insurance certificates, healthcare licenses, and policy manuals.

Program Readiness Review:

  • DHHS and BEAS evaluate provider readiness, including health care protocols, safety measures, and staff qualifications.

Approval & Medicaid Enrollment:

  • Upon approval, providers are authorized to bill Medicaid for Adult Health Services using designated billing codes.

Role: Licensed Practical Nurses (LPNs)Requirements: Active LPN license in New Hampshire; supervised by an RN; experience in direct patient care.

Role: Health Aides/Personal Care Workers

Requirements: Certification in personal care or healthcare support; experience in ADL assistance and health monitoring.

All staff must complete:
