2. DAY HABILITATION SERVICES OVERVIEW
Day Habilitation Services focus on developing skills that support independence, personal growth, and community inclusion.
Approved providers may deliver:
Skill Development: Training in personal hygiene, meal preparation, household chores, and budgeting.
Social and Community Integration: Facilitating group activities, community outings, and social interactions.

3. LICENSING & PROVIDER APPROVAL REQUIREMENTS
Prerequisites:
Register the business entity with the New Hampshire Secretary of State.
Obtain EIN from the IRS and NPI (Type 2).
Enroll as a Medicaid Waiver Day Habilitation provider via the New Hampshire Medicaid Provider Enrollment Portal.
Obtain licensure as a Day Habilitation Program from DHHS if applicable.
