If you're subcontracting under another agency, you're building someone else's business — here's how to bill in your own name.

Going from subcontractor to a directly enrolled Medicaid and managed care provider is a sequence of credentialing moves, and the order matters as much as the steps.

Fix your taxonomy first

Everything downstream depends on your NPPES record carrying the taxonomy that matches the services you deliver. If it's wrong or missing, applications stall later for reasons that trace back to this first step.

Enroll with Medicaid and CAQH

Complete your Medicaid provider enrollment so you're recognized in your state's system, and build a complete, current CAQH profile so managed care plans can credential you without endless back-and-forth.

Register and contract with each plan

Many states deliver services through a statewide managed care program you must join, and direct payment then requires a contract with each managed care organization you want to bill. Watch enrollment windows, because some states only open them certain months.

Key takeaway: Going direct is a sequence — correct taxonomy, then Medicaid and CAQH enrollment, then state registration, then a contract with each MCO. Start early and keep your current referrals flowing.
