2. SUPPORTED LIVING SERVICE OVERVIEW

Supported Living Services focus on helping individuals live as independently as possible while providing the necessary support to ensure safety and well-being. Services are tailored according to the Individual Service Plan (ISP) or Person-Centered Plan (PCP) and may include:

  • Personal care and assistance with activities of daily living (ADLs)

  • Health monitoring and medication management

  • Skills training for independent living (e.g., cooking, budgeting)

  • Community integration and social support

  • Transportation to appointments and community events

  • Safety supervision and crisis intervention

 

3. LICENSING & PROVIDER APPROVAL REQUIREMENTS

    • Background check and TB clearance

Training Requirements:
