Supported Living Services in Minnesota assist individuals with disabilities to live independently within their communities. These services offer personalized care, supervision, and skill-building support to enhance daily living and social integration. Supported Living Services are available under Minnesota’s Home and Community-Based Services (HCBS) Waivers.

Role: Licenses and regulates supported living facilities and care providers
Agency: Centers for Medicare & Medicaid Services (CMS)
Role: Provides federal oversight and ensures waiver compliance
2. SUPPORTED LIVING SERVICE OVERVIEW
Supported Living Services focus on helping individuals live as independently as possible while providing the necessary support to ensure safety and well-being. Services are tailored according to the Individual Service Plan (ISP) or Person-Centered Plan (PCP) and may include:
Personal care and assistance with activities of daily living (ADLs)
Health monitoring and medication management
Skills training for independent living (e.g., cooking, budgeting)

Transportation to appointments and community events

3. LICENSING & PROVIDER APPROVAL REQUIREMENTS
Prerequisites:
Register your business with the Minnesota Secretary of State
Obtain EIN from the IRS and NPI (Type 2)
Enroll as a Medicaid provider with the Minnesota Department of Human Services
Obtain a Comprehensive Home Care License from the Minnesota Department of Health
Maintain liability insurance and staff training documentation
