Illinois Medicaid Waivers: Who Runs Each Program — and How to Expand

Illinois operates nine Medicaid home and community-based services waivers — and if you plan to expand your agency into a new one, the operating agency, not just Medicaid, determines your path.

The Illinois Department of Healthcare and Family Services, HFS, is the single state Medicaid agency and administers all nine 1915(c) waiver programs. But HFS delegates day-to-day operations of eight of them to sister agencies, and that delegation decides who certifies you, what standards apply, and where your application goes.

Who runs what in Illinois

The Illinois Department on Aging operates the waiver for persons who are elderly. The DHS Division of Developmental Disabilities operates the waiver for adults with developmental disabilities. The DHS Division of Rehabilitation Services operates the waivers for persons with disabilities, brain injury, and HIV/AIDS. HFS itself operates the Supportive Living Program.

Why the operating agency matters

Certification does not transfer between operating agencies. Serving older adults under the Department on Aging does not qualify you to serve adults with developmental disabilities under DDD or persons with brain injury under DRS — each division maintains its own provider standards and approval process. Identify the operating agency behind your target waiver first, and confirm its current requirements before committing resources.

Enrollment runs through IMPACT

IMPACT is Illinois's online Medicaid provider enrollment system, run by HFS. Providers enroll and revalidate through IMPACT, and your enrollment record is what allows your claims to be paid. When you add a service line, plan for your IMPACT enrollment to be established or updated to match — and if your members are in managed care, treat MCO credentialing as its own workstream and confirm expectations with HFS.
