FMS providers act as fiscal intermediaries to ensure funds are used appropriately and in compliance with federal and state guidelines.

FMS is a required support in participant-directed models under Utah's HCBS Waivers and is administered by the Division of Services for People with Disabilities (DSPD). FMS providers handle payroll processing, tax withholdings, employee background checks, and financial reporting on behalf of waiver participants.

1. GOVERNING AGENCIES

Agency: Utah Department of Health and Human Services (DHHS) – Division of Services for People with Disabilities (DSPD)

Role: Contracts and monitors FMS providers under self-directed service options within HCBS Waivers.

Agency: Utah Division of Integrated Healthcare (DIH)

Role: Processes Medicaid claims and ensures financial accountability and reimbursement accuracy.

Agency: Internal Revenue Service (IRS)

Role: Provides Employer Identification Numbers (EINs) and enforces tax compliance for participant-employed workers.

 

2. FINANCIAL MANAGEMENT SERVICES OVERVIEW

FMS providers support individuals who choose to self-direct their Medicaid-funded services by handling the administrative and financial responsibilities that come with managing a budget and employing caregivers.

Key functions include:

  • Issuing payments to workers, vendors, and service providers

  • Handling employer-related taxes and withholdings

  • Assisting with budget monitoring and expenditure tracking

  • Conducting new hire paperwork and eligibility verifications

  • Performing required background checks on employees

  • Preparing and filing IRS and Utah state tax forms

  • Enroll with Utah Medicaid via the Provider Enrollment Portal

4. UTAH PROVIDER ENROLLMENT PROCESS

Step 4: Submit FMS Policy & Procedure Manual for approval

Requirements: Knowledge of participant-directed models, budget monitoring, and ISP-based spending

Role: Compliance Officer

Requirements: Oversight of HIPAA, IRS, and Medicaid program requirements

Support Staff: Background check processing, customer service, billing, and data entry

All staff must complete:

  • Medicaid and DSPD program training

  • IRS compliance and tax procedure training

  • HIPAA and financial data security training

  • Ongoing competency evaluations and policy refreshers

 

7. MEDICAID WAIVER PROGRAMS

FMS is required for self-directed services under:

  • Community Supports Waiver (CSW)

  • Acquired Brain Injury (ABI) Waiver

  • Physical Disabilities Waiver (PDW)

  • Medically Complex Children’s Waiver (MCCW)

Participants using employer authority or budget authority must be supported by an FMS provider. Authorization is reflected in the Individual Support Plan (ISP).
