2. CARE COORDINATION SERVICE OVERVIEW

Care Coordination Services ensure that individuals receiving Medicaid Waiver services have access to comprehensive care planning, service monitoring, and support with navigating complex health and community systems. The service is essential to delivering individualized, person-centered care.

Approved providers may deliver:

  • Person-centered assessment and service plan development

  • Regular monitoring and documentation of service outcomes

  • Coordination with waiver service providers and healthcare professionals

  • Advocacy for client rights and goal achievement

3. LICENSING & PROVIDER APPROVAL REQUIREMENTS

Prerequisites:

  • Register business with the Alaska Division of Corporations, Business and Professional Licensing

  • Obtain EIN from the IRS and NPI (Type 2)

  • Maintain an administrative office in Alaska with secure storage for records

  • Demonstrate compliance with SDS training and ethical standards for care coordination

  • Develop documentation systems for care plans, case notes, and monitoring reports
