Medicaid is a federal-state partnership. Within broad federal rules, each state designs its own waivers, sets its own provider requirements, and runs its own enrollment systems. As a result, launching a Medicaid waiver or HCBS program is state-specific by design — what works cleanly in one state can be a non-starter next door.
Why the variation exists
Federal law lets states request “waivers” of certain Medicaid rules so they can offer home- and community-based services tailored to their populations. Because each state writes its own waiver, the services covered, the eligibility criteria, the provider qualifications, the rates, and the application process all differ.
Where the differences show up
The same program can require different licenses depending on the state. Provider enrollment may run through the state agency in one place and through managed care plans in another. States vary in required staff credentials, training hours, and accreditation. Even the definition of “provider” — and what an approval grants you — can change at the border.
