3. LICENSING & PROVIDER APPROVAL REQUIREMENTS
Prerequisites:
Register your transportation company with the PA Department of State.
Obtain an EIN and a Type 2 NPI.
Apply for a PUC Operating Authority (if transporting for compensation).
