3. LICENSING & PROVIDER APPROVAL REQUIREMENTS

Prerequisites:

  • Register the business entity with the North Dakota Secretary of State.

  • Enroll as a Medicaid Waiver Personal Assistance provider via the North Dakota Medicaid Management Information System (MMIS).
