Enroll as a Medicaid Waiver Nursing Facility Transition provider via the New Mexico Medicaid Provider Enrollment Portal.
Maintain general liability and professional liability insurance.

4. NEW MEXICO PROVIDER ENROLLMENT PROCESS
Initial Interest and Application:
