Role: Reviews and approves service plans including medical supplies and ensures provider compliance under HCBS waiver guidelines
Agency: Alabama Medicaid Agency
Role: Regulates provider enrollment, DME supply authorizations, billing, and reimbursement for Medical Supply Services
Agency: Centers for Medicare & Medicaid Services (CMS)
Role: Provides federal oversight to ensure DME and medical supply services meet Medicaid regulations under the HCBS framework
2. MEDICAL SUPPLY SERVICE OVERVIEW
Medical Supply Services provide individuals with IDD access to durable medical equipment and health-related items that are essential for daily living, mobility, and wellness. These supplies must be included in the person-centered service plan and medically justified. Approved providers may deliver:
Durable medical equipment (e.g., walkers, wheelchairs, hospital beds)
Incontinence products, wound care supplies, and adaptive aids
Nutritional supplements (with physician prescription)
Respiratory equipment and CPAP accessories
Home safety items (e.g., shower chairs, grab bars)
Routine delivery, training on equipment usage, and basic maintenance
All products must be cost-effective, pre-approved by Medicaid (if required), and documented to ensure they support the individual’s independence and health in their home environment.
3. LICENSING & PROVIDER APPROVAL REQUIREMENTS
Prerequisites:
Register business with the Alabama Secretary of State
Obtain EIN from the IRS and NPI (Type 2)
Obtain a DME license or accreditation from a CMS-recognized accrediting organization (if required)
Maintain business liability insurance and physical inventory tracking systems
Contract with manufacturers and distributors of Medicaid-approved medical supplies
Implement HIPAA-compliant order tracking, billing, and customer service processes
4. ADMH PROVIDER ENROLLMENT PROCESS
Initial Interest and Application:
Request the Medical Supply Services Provider Application Packet from ADMH
Submit a business plan outlining inventory control, delivery procedures, and product categories offered
Include policies for verifying medical necessity, coordinating with service teams, and documenting supply issuance
Background & Credentialing:
Background checks for staff handling consumer data or making in-home deliveries
Proof of appropriate DME licensure and CMS accreditation (if required)
Documentation of staff training in customer service, product usage, and HIPAA compliance
Program Readiness Review:
Review of billing systems, order tracking logs, and product return policies
Sample documentation including delivery receipts, consumer consent forms, and physician orders
Emergency response protocols for urgent supply needs
Approval & Medicaid Enrollment:
Once approved by ADMH, enroll with Alabama Medicaid as a Medical Supply Services provider
Add applicable billing codes for DME and supply items

Role: Delivery Technician / Fulfillment Staff
Requirements: High school diploma or GED; training in equipment handling, customer service, and HIPAA compliance
All staff must complete:
ADMH orientation and HCBS waiver training
Annual ethics, confidentiality, and safety training
Training on medical supply handling, documentation, and consumer interaction
7. MEDICAID WAIVER SERVICES
The following Alabama Medicaid Waivers authorize Medical Supply Services:

LAH Waiver (Living at Home Waiver)
State of Alabama Community Waiver Program
Approved providers may deliver:
