PROMOTING HEALTH AND INDEPENDENCE THROUGH NUTRITIONAL SUPPORT AND HOME-DELIVERED MEALS FOR INDIVIDUALS WITH DISABILITIES
Meal & Nutrition Services in Arizona help eligible individuals maintain their health, independence, and well-being by ensuring access to nutritious meals. Provided under the Arizona Long Term Care System (ALTCS) and Home and Community-Based Services (HCBS) Waivers, these services support individuals who may be unable to prepare their own meals or who require special dietary accommodations.
1. GOVERNING AGENCIES
Agency: Arizona Health Care Cost Containment System (AHCCCS)
Role: Administers Medicaid funding and ensures nutritional services meet medical necessity and HCBS guidelines
Agency: Arizona Department of Economic Security (DES) – Division of Developmental Disabilities (DDD)
Role: Authorizes vendors to provide meals for individuals with developmental disabilities
Agency: Centers for Medicare & Medicaid Services (CMS)
Role: Enforces federal Medicaid standards and oversees waiver service compliance
2. MEAL & NUTRITION SERVICE OVERVIEW
Meal & Nutrition Services provide direct access to regular, healthy meals that support dietary needs and overall wellness. Services may be delivered in the home or at a designated site and are often tailored to accommodate health conditions or disability-related limitations.
Approved providers may deliver:
Home-delivered meals (daily or weekly delivery)
Congregate meals (if offered at program locations)
Nutritionally balanced menus designed by a registered dietitian
Specialized meals for diabetes, renal, low-sodium, or allergy-sensitive diets
Education on food preparation and nutrition (as applicable)
Documentation of delivery, meal preferences, and dietary compliance
3. LICENSING & PROVIDER APPROVAL REQUIREMENTS
Prerequisites:
Register your business with the Arizona Corporation Commission
Obtain EIN from the IRS and NPI (Type 2)
Apply through the Qualified Vendor Application and Directory System (QVADS) (if serving DDD members)
Meet local food safety and handling regulations (Maricopa County or local jurisdiction)
Maintain kitchen inspection records, food handler certifications, and delivery logs
Hire or contract with a registered dietitian (for menu development and compliance)
4. DDD QUALIFIED VENDOR ENROLLMENT PROCESS
Step 1: Orientation and Application
Attend DDD Qualified Vendor orientation
Submit QVADS application, including service plan, dietary protocols, and delivery systems
Step 2: Documentation Submission
Include kitchen licenses, sample menus, and food safety training records
Submit intake and tracking forms for delivery and consumer satisfaction
Step 3: Contract Execution
Once approved, provider signs Qualified Vendor Agreement

6. STAFFING REQUIREMENTS
Role: Nutrition Program Coordinator / Supervisor
Requirements: Experience in food service, health programs, or meal planning; ServSafe Manager or equivalent
Role: Meal Prep Staff / Drivers
Requirements:

Clean driving record and background check
Knowledge of dietary restrictions and safe food transport
