3. LICENSING & PROVIDER APPROVAL REQUIREMENTS
Prerequisites:
Register the business entity with the Nebraska Secretary of State.

Enroll as a Medicaid Waiver Homemaker Services provider via the Nebraska Medicaid Provider Enrollment Portal.
