CREATING SAFE, ACCESSIBLE, AND FUNCTIONAL LIVING ENVIRONMENTS TO SUPPORT INDEPENDENCE AND COMMUNITY INTEGRATION
Home Modification Services in Wyoming assist individuals with disabilities, chronic health conditions, or age-related mobility limitations by making physical changes to their homes. These changes improve accessibility, reduce injury risks, and support independent living. Modifications may range from minor safety enhancements to major structural changes, based on the individual's functional needs. These services are covered under Wyoming Medicaid’s Home and Community-Based Services (HCBS) Waivers, including the Comprehensive and Supports Waivers for individuals with developmental disabilities, and the Community Choices Waiver (CCW) for elderly or physically disabled adults. All modifications must be identified in the participant’s Individualized Plan of Care (IPC), assessed for necessity, and approved prior to installation.
