2. HOME MODIFICATION SERVICE OVERVIEW

Approved providers may deliver:
Installation of wheelchair ramps, stair lifts, and widened doorways
Bathroom modifications such as roll-in showers, grab bars, and accessible sinks
Kitchen modifications, including lowered counters and accessible appliances
Structural adjustments to improve mobility within the home
Installation of handrails, non-slip flooring, and emergency alert systems
Environmental control unit setup (lighting, temperature, entry systems)
All modifications must directly support needs identified in the participant’s Individualized Service Plan (ISP) and may not include general home improvements or repairs unrelated to the participant’s disability.
3. LICENSING & PROVIDER APPROVAL REQUIREMENTS
Prerequisites:
Register your business with the Hawaii Department of Commerce and Consumer Affairs (DCCA)
Obtain EIN from the IRS and NPI (Type 2)
Hold an appropriate contractor’s license in Hawaii (e.g., C-5 Cabinet, Millwork and Finish Carpentry Contractor; C-15 Electronic Systems Contractor; or General Contractor license, depending on scope)
Enroll as a Medicaid provider through Hawaii’s Med-QUEST Division
Maintain commercial general liability, professional liability, and workers’ compensation insurance
Develop participant-centered policies for project assessment, execution, and safety
4. MED-QUEST/DHS PROVIDER ENROLLMENT PROCESS
Initial Interest and Application:
Submit Provider Enrollment Application identifying Home Modification Services under Hawaii’s HCBS programs
Provide contractor licensing and insurance verification
Application and Documentation Submission:
Submit Articles of Incorporation, proof of EIN/NPI, contractor’s license(s), insurance certificates, and operational policies
