2. HOME MODIFICATION SERVICE OVERVIEW

Home Modification Services involve permanent or semi-permanent changes to a participant’s residence to remove barriers and enhance independence. All modifications must be directly related to the individual’s disability, pre-approved by DDDS, and documented in the ISP.

Approved providers may deliver:

Installation of wheelchair ramps and stair lifts
Roll-in showers, grab bars, and raised toilets
Widening of doorways and hallways
Lowering counters, sinks, and light switches
Installation of visual/auditory alert systems
Modified flooring and non-slip surfaces
Pre-project assessments, construction, and post-installation documentation

3. LICENSING & PROVIDER APPROVAL REQUIREMENTS

Prerequisites:

Register your business with the Delaware Division of Corporations
Obtain EIN from the IRS and NPI (Type 2)
Hold a valid Delaware contractor’s license or subcontract licensed professionals
Apply as a Home Modification Services Provider under DDDS
