2. HOME MODIFICATION SERVICE OVERVIEW

Home Modification Services involve permanent or semi-permanent changes to a participant’s residence to remove barriers and enhance independence. All modifications must be directly related to the individual’s disability, pre-approved by DDDS, and documented in the ISP.

Approved providers may deliver:

  • Installation of wheelchair ramps and stair lifts

  • Roll-in showers, grab bars, and raised toilets

  • Widening of doorways and hallways

  • Lowering counters, sinks, and light switches

  • Installation of visual/auditory alert systems

  • Modified flooring and non-slip surfaces

  • Pre-project assessments, construction, and post-installation documentation

3. LICENSING & PROVIDER APPROVAL REQUIREMENTS

Prerequisites:

  • Register your business with the Delaware Division of Corporations

  • Obtain EIN from the IRS and NPI (Type 2)

  • Hold a valid Delaware contractor’s license or subcontract licensed professionals

  • Apply as a Home Modification Services Provider under DDDS

  • Carry general liability and product liability insurance

  • Maintain HIPAA-compliant documentation systems
