Home Modification Services in Arizona help individuals with disabilities maintain independence by making structural changes to their homes. 2. HOME MODIFICATION SERVICE OVERVIEW

Home Modification Services include physical adaptations to an individual’s primary residence to ensure accessibility, safety, and continued community living. These modifications must be medically necessary and align with the participant’s person-centered service plan.

Approved providers may deliver:

  • Installation of wheelchair ramps and grab bars

  • Widening of doorways and hallways

  • Roll-in shower conversions or accessible tubs

  • Lowering of countertops or cabinetry

  • Installation of stair lifts, handrails, and non-slip flooring

  • Modifications to improve access to bedrooms, bathrooms, and entrances/exits

  • Coordination with contractors, inspectors, and funding sources

  • Documentation of completed work and client satisfaction

3. LICENSING & PROVIDER APPROVAL REQUIREMENTS

Prerequisites:

  • Register business with the Arizona Corporation Commission

  • Obtain EIN from the IRS and NPI (Type 2)

  • Apply to become a Qualified Vendor through the Qualified Vendor Application and Directory System (QVADS) (if serving DDD members)

  • Obtain necessary local construction licenses and insurance

  • Hire licensed and bonded contractors or subcontractors

  • Maintain general liability and workers' compensation insurance

  • Develop service tracking and documentation systems

4. DDD QUALIFIED VENDOR ENROLLMENT PROCESS

Step 1: Orientation and Pre-Application

  • Attend the DDD Qualified Vendor orientation

  • Review policies on home safety, project scope, and allowable modifications

Step 2: Submit QVADS Application

  • Include documentation such as:

    • Business and service plan

    • Contractor licensing and bonding information

    • Sample modification approval and completion forms

    • Safety inspection protocols and participant feedback forms

Step 3: Review and Agreement

  • Upon approval, provider signs a Qualified Vendor Agreement

  • Services are listed in the Qualified Vendor Directory

5. REQUIRED DOCUMENTATION

  • Policy & Procedure Manual including:

    • Intake and assessment coordination

    • Incident prevention and emergency response

    • Informed consent, project tracking, and quality control

    • Client rights and grievance procedures

    • Documentation of cost estimates, before/after photos, and sign-off forms

6. STAFFING REQUIREMENTS

Role: Home Modification Project Coordinator

Requirements: Experience with home renovations, ADA compliance, or disability services

Role: Contractor / Construction Technician

Requirements:

  • Arizona general contractor license or subcontractor agreement

  • Bonded and insured

  • Background check (especially if working in participant homes)

Training Requirements for All Staff:
