2. HOME HEALTH SERVICE OVERVIEW
Home Health Services provide skilled care and health-related assistance in an individual’s residence. These services must be ordered by a physician or included in the person’s care plan and are delivered by licensed professionals under the supervision of a registered nurse or medical director.
Approved providers may deliver:
Skilled nursing care (e.g., wound care, injections, catheter care, medication administration)
Licensed Practical Nurse (LPN) services under RN supervision

3. LICENSING & PROVIDER APPROVAL REQUIREMENTS
Prerequisites:
Register business with the Arizona Corporation Commission
Obtain EIN from the IRS and NPI (Type 2)
Apply for a Home Health Agency License through the Arizona Department of Health Services (ADHS)
Apply through the Qualified Vendor Application and Directory System (QVADS) (if serving DDD members)
