Home Health Services in Arizona provide medically necessary care and therapeutic support to individuals in their home setting. Under the Arizona Long Term Care System (ALTCS) and Home and Community-Based Services (HCBS) Waivers, providers deliver these services through licensed home health agencies and Medicaid-authorized staff.

1. GOVERNING AGENCIES

Agency: Arizona Department of Health Services (ADHS) – Bureau of Medical Facilities Licensing

Role: Licenses home health agencies and ensures compliance with medical care standards and safety protocols

Agency: Arizona Health Care Cost Containment System (AHCCCS)

Role: Oversees Medicaid enrollment, reimbursement, and program compliance

Agency: Arizona Department of Economic Security (DES) – Division of Developmental Disabilities (DDD)

Role: Coordinates waiver services for individuals with developmental disabilities and manages Qualified Vendor authorizations

Agency: Centers for Medicare & Medicaid Services (CMS)

Role: Provides federal oversight for home health services under Medicaid and HCBS waivers

 

2. HOME HEALTH SERVICE OVERVIEW

Home Health Services provide skilled care and health-related assistance in an individual’s residence. These services must be ordered by a physician or included in the person’s care plan and are delivered by licensed professionals under the supervision of a registered nurse or medical director.

Approved providers may deliver:

  • Skilled nursing care (e.g., wound care, injections, catheter care, medication administration)

  • Licensed Practical Nurse (LPN) services under RN supervision

  • Home Health Aide services for ADL support and basic personal care

  • Physical, occupational, or speech therapy (if offered)

  • Medical monitoring and care coordination

  • Health education and support for families or caregivers

  • Documentation of visits, vital signs, interventions, and physician coordination

3. LICENSING & PROVIDER APPROVAL REQUIREMENTS

Prerequisites:

  • Register business with the Arizona Corporation Commission

  • Obtain EIN from the IRS and NPI (Type 2)

  • Apply for a Home Health Agency License through the Arizona Department of Health Services (ADHS)

  • Apply through the Qualified Vendor Application and Directory System (QVADS) (if serving DDD members)

  • Maintain liability and malpractice insurance

  • Hire licensed nurses and clinical staff

  • Develop HIPAA-compliant documentation and care tracking systems

 

4. LICENSING & MEDICAID ENROLLMENT PROCESS

Step 1: ADHS Licensing

  • Submit a Home Health Agency license application

  • Prepare for an inspection covering documentation, staff files, safety procedures, and clinical protocols

  • Submit administrative policies, staffing plans, and client rights documentation

Step 2: AHCCCS Enrollment

  • Register as a Medicaid provider through AHCCCS

  • Submit license, NPI, and staff credential documentation

  • Set up AHCCCS Online portal access for claims submission

Step 3 (if applicable): QVADS Application for DDD

  • Submit staffing plans, documentation templates, and agency profile

  • Complete contract approval and receive vendor listing

7. MEDICAID WAIVER SERVICES

Home Health Services are authorized under:

  • ALTCS – Elderly and Physically Disabled (EPD) Waiver

  • ALTCS – Division of Developmental Disabilities (DDD) Waiver

  • Children’s Rehabilitative Services (CRS)

  • Arizona Early Intervention Program (AzEIP)

Approved providers may deliver:

  • Intermittent skilled nursing and therapy services

  • In-home health aide support

  • Ongoing documentation and coordination with the care team

  • Monitoring of chronic conditions and early detection of health concerns
