SUPPORTING FAMILIES TO MAINTAIN CAREGIVING ROLES, PREVENT CRISIS, AND PROMOTE LONG-TERM WELL-BEING
Family Stability Assistance Services in Ohio provide targeted support to families caring for individuals with disabilities or complex needs. 2. FAMILY STABILITY ASSISTANCE SERVICES OVERVIEW

Approved providers may deliver:
Caregiver Training and Education: Providing instruction on medical care, behavior management, and emergency preparedness.
Family Counseling and Mediation: Supporting family communication and stress management.
Crisis Support Planning: Helping families develop tools to navigate emergencies without institutionalization.
In-Home Consultation: Professionals guiding families on daily routines, safety strategies, and community resource navigation.
Behavioral Coaching: Teaching strategies for caregivers to manage challenging behaviors.
Community Resource Linkage: Connecting families with peer support, housing assistance, or other aid.
Family Respite Coordination: Referring to or integrating respite options to prevent caregiver fatigue.
Supportive Technology Access: Tools to help families monitor or assist loved ones at home.

3. LICENSING & PROVIDER APPROVAL REQUIREMENTS
Prerequisites:
Register your business with the Ohio Secretary of State.
Obtain an EIN from the IRS and NPI (Type 2).
Enroll via the DODD Provider Certification Wizard or Medicaid Provider Portal (ODM).
Maintain general and professional liability insurance.
Develop written policies for family engagement, service planning, emergency response, and documentation.
Ensure all support staff and facilitators pass background checks and complete caregiver support training.
4. OHIO PROVIDER ENROLLMENT PROCESS
Initial Application:
Submit an application to DODD or ODM (depending on waiver program alignment).

6. STAFFING REQUIREMENTS
Role: Family Stability Services Director
Requirements: Bachelor’s or Master’s in social work, psychology, or family counseling; experience in home-based or disability-related support.
Role: Family Support Coaches / Case Workers
Requirements: Background in human services, behavioral health, or waiver coordination; caregiver training; CPR/First Aid certified.
Role: Clinical Consultants (if applicable)
Requirements: Licensed clinicians or behavior specialists to assist with complex family dynamics or trauma-informed planning.
All staff must complete:
Training in person-centered planning, crisis prevention, and caregiver support
HIPAA and rights-based service delivery education
Ongoing continuing education and annual competency reviews
