PROMOTING INDEPENDENT LIVING, RELATIONSHIP-BUILDING, AND ACTIVE PARTICIPATION IN THE COMMUNITY FOR INDIVIDUALS WITH DISABILITIES
Community Integration Services in Delaware support individuals with intellectual and developmental disabilities (IDD) in accessing their communities through individualized, goal-driven activities that build social connections, independence, and personal fulfillment. These services are authorized under the Delaware Lifespan Waiver, administered by the Division of Developmental Disabilities Services (DDDS), and align with the participant’s Individual Service Plan (ISP).
1. GOVERNING AGENCIES
Agency: Delaware Division of Developmental Disabilities Services (DDDS)
Role: Authorizes and monitors the delivery of Community Integration Services through the Lifespan Waiver
Agency: Delaware Division of Medicaid and Medical Assistance (DMMA)
Role: Administers Medicaid reimbursement and waiver oversight
Agency: Centers for Medicare & Medicaid Services (CMS)
Role: Provides federal oversight to ensure HCBS services promote inclusion and person-centered outcomes
2. COMMUNITY INTEGRATION SERVICE OVERVIEW
Community Integration Services help individuals with disabilities build meaningful relationships, access local resources, and engage in public life. Services are tailored to each participant’s interests, needs, and ISP goals, with a focus on choice, dignity, and participation.
Approved providers may deliver:
Support with accessing libraries, recreational centers, and events
Assistance with joining clubs, volunteer groups, or social organizations
Skill-building for navigating public transportation and community safety
Support in developing relationships and self-advocacy
Exploration of hobbies, wellness activities, and cultural experiences
Documentation of outcomes and progress toward community goals
3. LICENSING & PROVIDER APPROVAL REQUIREMENTS
Prerequisites:
Register your business with the Delaware Division of Corporations
Obtain EIN from the IRS and NPI (Type 2)
Apply as a Community Integration Services Provider through DDDS
Employ qualified Direct Support Professionals (DSPs) trained in person-centered planning
Carry general liability insurance and comply with HIPAA documentation standards
Develop transportation safety procedures (if providing travel support)
4. PROVIDER ENROLLMENT PROCESS
Step 1: Register your business and obtain an NPI
Step 2: Submit a provider application to DDDS for Community Integration Services
Include staff resumes, service descriptions, policies, and sample documentation
Step 3: Enroll with Delaware Medicaid (DMAP) if billing directly
Step 4: Coordinate with support coordinators for referrals and ISP alignment
Step 5: Deliver services in the community and submit documentation as required

6. STAFFING REQUIREMENTS
Role: Direct Support Professional (DSP)
Requirements:
High school diploma or equivalent
Background check, CPR/First Aid certification
Experience supporting individuals with disabilities in community settings preferred
Role: Program Coordinator / Supervisor
Requirements:
Oversight of planning, documentation, and staff performance
Experience in HCBS waiver programs or community services
Training Requirements for All Staff:
Person-centered planning and community engagement
HIPAA and participant confidentiality
Abuse prevention and mandated reporting
Transportation and travel safety
Documentation and ISP tracking
7. MEDICAID WAIVER SERVICES
Community Integration Services are reimbursed under:
Delaware Lifespan Waiver (administered by DDDS)
Approved providers may deliver:
Community-based 1:1 or group supports
Assistance with social engagement, personal goals, and public resource access
Progress documentation and communication with support teams
Person-centered programming aligned with the ISP
