2. CASE MANAGEMENT SERVICES OVERVIEW

3. LICENSING & PROVIDER APPROVAL REQUIREMENTS

Enroll as a Medicaid Waiver Case Management provider via the New York Medicaid Provider Enrollment Portal (eMedNY).

4. NEW YORK PROVIDER ENROLLMENT PROCESS
