PROMOTING WELLNESS AND INDEPENDENCE THROUGH NON-INSTITUTIONAL HEALTH SUPPORT FOR ADULTS WITH CHRONIC ILLNESSES AND DISABILITIES
Adult Health Services in Arizona are designed to support adults who need routine health monitoring, therapeutic supervision, and assistance with managing chronic conditions, while remaining in a home or community-based setting. These services are part of Arizona’s Long Term Care System (ALTCS) and are delivered through Home and Community-Based Services (HCBS) to delay or prevent institutional care.
1. GOVERNING AGENCIES
Agency: Arizona Health Care Cost Containment System (AHCCCS)
Role: Administers Arizona Medicaid and oversees service reimbursements and provider enrollment
Agency: Arizona Department of Economic Security (DES) – Division of Developmental Disabilities (DDD)
Role: Approves Qualified Vendors and monitors Adult Health Service delivery for individuals with developmental disabilities
Agency: Centers for Medicare & Medicaid Services (CMS)
Role: Ensures federal compliance with HCBS and person-centered service standards
2. ADULT HEALTH SERVICE OVERVIEW
Adult Health Services help individuals manage chronic health conditions and improve quality of life through wellness-focused supports. These services are non-emergency and must be included in the person’s care or support plan.
Approved providers may deliver:
Health-related supports and condition monitoring
Non-clinical assistance with mobility, personal hygiene, and nutrition
Health education and chronic illness self-management
Supervision to maintain safety and avoid health deterioration
Routine wellness checks and documentation of health status
Coordination with physicians and support coordinators as needed
3. LICENSING & PROVIDER APPROVAL REQUIREMENTS
Prerequisites:
Register your business with the Arizona Corporation Commission
Obtain EIN from the IRS and NPI (Type 2)
Apply through the Qualified Vendor Application and Directory System (QVADS)
Maintain liability insurance and staff background checks
Develop written policies that support person-centered health monitoring
Hire trained staff (CNA, DSP, or wellness aides depending on setting and plan)
4. DDD QUALIFIED VENDOR ENROLLMENT PROCESS
Step 1: Pre-Application & Orientation
Attend the required DDD Qualified Vendor orientation
Review adult health service scope and delivery guidelines
Step 2: QVADS Application Submission
Submit documentation via QVADS including:
Staff credentials and training protocols
Sample health monitoring logs and progress note templates
Emergency policies, wellness protocols, and safety systems
Step 3: Approval & Contracting
Upon review, providers sign a Qualified Vendor Agreement
Services and rates will be listed in the Qualified Vendor Directory

6. STAFFING REQUIREMENTS
Role: Adult Health Program Supervisor
Requirements: Background in nursing, public health, or direct care supervision
Role: Wellness Assistant / Direct Support Staff
Requirements:
High school diploma or GED
CPR/First Aid certification
Background check and Article 9 training (if serving DDD)
CNA certification (optional but preferred for enhanced services)
Training Requirements for All Staff:

Annual refresher and compliance training
7. MEDICAID WAIVER SERVICES
The following Arizona Medicaid Waivers authorize Adult Health Services:
ALTCS – Elderly and Physically Disabled (EPD) Waiver
ALTCS – Division of Developmental Disabilities (DDD) Waiver
Approved providers may deliver:
Non-clinical health supervision and daily wellness checks
Nutrition support, hygiene assistance, and personal care
Chronic illness support based on care plan goals
Coordination with other service providers and reporting to case managers
