3. LICENSING & PROVIDER APPROVAL REQUIREMENTS

  • Enroll as a Medicaid Waiver Adult Day Services provider via the North Dakota Medicaid Management Information System (MMIS).

4. NORTH DAKOTA PROVIDER ENROLLMENT PROCESS
