2. ADAPTIVE EQUIPMENT SERVICES OVERVIEW

3. LICENSING & PROVIDER APPROVAL REQUIREMENTS

Enroll via the DODD Provider Certification Portal or Ohio Medicaid’s Provider Enrollment system.
