Wyoming - Skilled Respite Service — Licensing, Medicaid Enrollment and Startup Requirements

Last reviewed: 2026-09-10

The Wyoming Department of Health (WDH) Division of Healthcare Financing authorizes Skilled Respite services through the Community Choices Waiver (CCW) and the Comprehensive and Supports Waivers (DD). Providers must first secure access to the Wyoming Health Provider (WHP) portal and complete mandatory state-developed training modules before an initial HCBS certification application is even generated for their agency.

Because this service requires the delivery of care by licensed nursing staff for participants with complex medical needs, applicants must hold active Wyoming nursing licenses (RN or LPN) or operate as a licensed Home Health Agency prior to seeking HCBS waiver certification. Once certified by the HCBS section, the provider must separately enroll in the Wyoming Medicaid Discover Your Provider (DyP) system to receive a Medicaid Provider Agreement.

1. Service Definition and Scope

In Wyoming, Skilled Respite is a specialized tier of respite care provided under the state's Medicaid HCBS waivers, specifically targeting participants whose medical or behavioral conditions require oversight by licensed nursing personnel. It provides temporary relief to the primary unpaid caregiver while ensuring the participant's complex health needs are safely managed.

This service is distinct from basic respite or personal care, as it mandates the clinical competencies of a Registered Nurse (RN) or Licensed Practical Nurse (LPN). It can be delivered in the participant's home or in an approved facility setting, depending on the specific waiver's service definitions.

2. Regulatory and Oversight Agencies

The Wyoming Department of Health (WDH) is the primary umbrella agency, with specific divisions handling different phases of the approval and oversight process. The Division of Healthcare Financing manages the HCBS waiver programs and Medicaid enrollment.

Nursing licensure, which is a prerequisite for delivering skilled respite, is overseen by the Wyoming State Board of Nursing. Facility-based providers or Home Health Agencies must also interact with the WDH Healthcare Licensing and Surveys division.

3. Gatekeeping Prerequisites: Who Can Even Apply

Wyoming utilizes a closed-portal application system for HCBS providers. An applicant cannot simply download an application; they must first request and be granted access to the Wyoming Health Provider (WHP) portal. This access request is manually reviewed and takes up to 48 hours for approval.

Furthermore, to provide Skilled Respite, the applicant must already possess the underlying professional or facility licensure required to deliver nursing services in Wyoming. An individual must hold an active RN or LPN license, or an agency must be licensed as a Home Health Agency, before the HCBS certification will be granted.

4. Licensure and Certification Requirements

Providers must achieve HCBS Certification through the WDH HCBS Section. This process is entirely digital and managed through the WHP portal. Applicants must submit proof of their underlying nursing or home health licensure alongside their HCBS application.

The certification process requires strict adherence to the CCW WHP File Naming Conventions. Any uploaded document that does not follow this exact naming structure will result in the application being returned or denied.

5. Medicaid Provider Enrollment

After receiving HCBS Certification, the provider must enroll in Wyoming Medicaid to bill for services. This is done through the Discover Your Provider (DyP) portal hosted by HTG Technology. Providers must select the correct PRESM Provider Type during this enrollment phase.

The Medicaid Provider Agreement is a separate legal contract from the HCBS certification. Both the certification and the Medicaid enrollment must be maintained and revalidated every five years.

6. Staffing, Training and Background Checks

Wyoming mandates specific state-produced training for all HCBS providers before certification. A representative from each provider agency must complete recorded modules on the HCBS Settings Rule and Person-Centered Planning, retaining a Training Summary Form for each.

All staff providing direct care must pass a multi-step background screening. Because Skilled Respite involves nursing tasks, staff must also maintain their clinical competencies and active nursing licenses in good standing with the state board.

7. Documentation, Policies and Records

Providers must maintain comprehensive records demonstrating compliance with both HCBS waiver rules and nursing standards of practice. This includes detailed care plans, medication administration records (MARs), and nursing notes for every respite shift.

During the application phase, the most critical documentation rule is the file naming convention. The state explicitly requires all files uploaded to the WHP portal to be renamed according to their published guidelines before submission.

8. Billing, Rates and Claims

Skilled Respite is billed to Wyoming Medicaid using specific HCPCS codes and modifiers that designate the service as a waiver service provided by licensed nursing staff. Claims are submitted through the DyP portal or via an approved clearinghouse.

Rates for CCW and DD waiver services are established by the WDH Division of Healthcare Financing following periodic rate studies. Providers must accept the Medicaid rate as payment in full and cannot balance-bill the participant.

9. Approval Sequence and Timeline

The approval sequence begins with requesting WHP portal access, which takes up to 48 hours. Once access is granted, the provider has exactly 90 days to complete trainings, gather documents, and submit the application.

If the application is not submitted within 90 days, it is automatically canceled and the provider must start over. After HCBS certification is granted, the provider proceeds to the DyP portal for Medicaid enrollment, which adds several weeks to the total timeline.

10. Common Denials and Survey Findings

A frequent cause for application denial or return in Wyoming is the failure to follow the CCW WHP File Naming Conventions. The state's portal system relies on these conventions for processing, and non-compliant files halt the review.

Another common issue is application cancellation due to the 90-day timeout rule. Providers who initiate the application before completing their background checks or trainings often run out of time and lose their progress.

11. Key Contacts and Resources

The primary point of contact for becoming a Skilled Respite provider under the waivers is the WDH HCBS Credentialing Team. They manage the WHP portal and the initial certification process.

For issues related to the Medicaid Provider Agreement or billing, providers must contact the Wyoming Medicaid fiscal agent through the DyP portal.
