You cannot simply apply for a license or enroll in Medicaid as a standalone employment provider; you must first successfully respond to a DSPD Request for Statement of Qualifications (RFSQ) and be awarded a contract to join the DSPD provider network. Without an active DSPD contract, any Medicaid enrollment application submitted for this service will be immediately rejected.

If an agency also provides facility-based day treatment or residential services, they must obtain the corresponding Human Services license from the DHHS Office of Licensing. However, for pure community-based Supported Employment, the DSPD contract and certification serve as the operating authority.

Once the DSPD contract is awarded, the provider must enroll in Utah Medicaid using the Provider Reimbursement Information System for Medicaid (PRISM). PRISM is a fully electronic, multi-factor authenticated portal.

Providers enroll as an HCBS Waiver Provider. The application requires uploading the DSPD contract, business licenses, and ownership disclosure forms to comply with federal Medicaid integrity regulations.

6. Staffing, Training and Background Checks

Staff providing Supported Employment (Job Coaches and Job Developers) must meet specific competency and background requirements to ensure the safety and success of waiver participants.

Utah places a strong emphasis on professionalizing employment supports, often requiring or strongly encouraging national certifications for staff who develop jobs or negotiate with employers.

Medicaid requires rigorous documentation to justify the billing of Supported Employment services. Every billed unit must be backed by a contemporaneous note detailing the intervention.

8. Billing, Rates and Claims

Supported Employment is billed on a fee-for-service basis through the PRISM MMIS system. Rates are standardized and established by the Utah Legislature and DSPD.

Providers cannot bill for services until a Prior Authorization (PA) has been loaded into PRISM by the client's DSPD Support Coordinator. Billing without a matching PA will result in an automatic claim denial.

Becoming a Supported Employment provider in Utah is a sequential process that can take several months, largely dependent on the DSPD RFSQ cycle.
