4. Licensure and Certification Requirements

Because Day Habilitation is not a licensed medical day care (Adult Day Health Services), it is governed by certification standards outlined in the Supports Program and Community Care Program Policies and Procedures Manuals. Providers undergo audits using the Day Habilitation Certification Auditing Tool.

Facilities must meet physical plant standards for safety, accessibility, and age-appropriate programming environments as defined by DDD.

5. Medicaid Provider Enrollment

After receiving provisional certification, providers complete the Combined Application to Become a Medicaid/DDD Approved Provider online. Hard copy packets must then be mailed to Gainwell Technologies in Trenton.

The enrollment packet requires specific state forms, tax documentation, and an authorized signature to establish the agency as a billing entity in the NJMMIS system.

6. Staffing, Training and Background Checks

Agency staff must meet qualifications set by DDD in the waiver manuals. This includes mandatory background checks and completion of DDD-mandated training modules before rendering services.

Agencies must ensure all applicable staff have current New Jersey licenses or certifications if mandated by their specific profession.

Providers must maintain comprehensive records aligning with the participant's New Jersey Individual Service Plan (NJISP). A formal Business Associate Agreement (BAA) with DHS is required to handle protected health information.

Reimbursement is issued under the DDD Fee-for-Service (FFS) system based on published rate schedules for the Supports and Community Care Programs.

9. Approval Sequence and Timeline

The approval process is strictly sequential, starting with DDD policy review and ending with Gainwell Medicaid activation. The facility review alone takes up to 45 days to schedule after the initial interview.

Providers cannot skip steps; the Combined Application will not be processed without the provisional certification already in hand.

10. Common Denials and Survey Findings

Applications are frequently delayed or denied due to incomplete packets or failure to pass the initial physical plant review. Falsification of any information during the application process is immediate cause for denial.

Gainwell will reject packets that are missing required tax documents or the signed Agreement of Understanding.
