Idaho - Day Habilitation Services — Licensing, Medicaid Enrollment and Startup Requirements

Last reviewed: 2026-08-16

In Idaho, Day Habilitation services are provided under the Adult Developmental Disabilities Waiver and the State Plan Option to help participants acquire, retain, or improve self-help, socialization, and adaptive skills in non-residential settings. To offer these services, an agency must be certified by the Idaho Department of Health and Welfare (IDHW) as a Developmental Disability Agency (DDA) and subsequently enroll as an Idaho Medicaid provider.

The single biggest structural barrier to entry for this service in Idaho is a strict, state-mandated moratorium on new provider applications. The IDHW Division of Licensing and Certification has officially paused all applications to become a new DDA or Residential Habilitation provider until September 1, 2026. No new applications will be accepted or reviewed until this pause is lifted.

1. Service Definition and Scope

Day Habilitation in Idaho is defined under IDAPA 16.03.10.326 as assistance with the acquisition, retention, or improvement in self-help, socialization, and adaptive skills. These services must take place in a non-residential setting, separate from the participant's private home or residential habilitation facility.

The service is designed to enable adults with developmental disabilities to attain or maintain their maximum functional level. It is distinct from supported employment and cannot be billed concurrently with residential habilitation or employment services for the same hours of service.

2. Regulatory and Oversight Agencies

The Idaho Department of Health and Welfare (IDHW) is the umbrella agency responsible for both the certification of facilities and the administration of the Medicaid program. Within IDHW, distinct divisions handle the regulatory oversight, background clearances, and financial enrollment of providers.

Medicaid claims processing and provider portal management are contracted out to Gainwell Technologies, which operates the Medicaid Enterprise System (MES) known as Health PAS OnLine.

3. Gatekeeping Prerequisites: Who Can Even Apply

Idaho enforces a hard structural barrier for new Day Habilitation providers. The state has implemented a complete moratorium on new Developmental Disability Agency (DDA) certifications. Until this moratorium expires, no new entities can enter the market.

For entities planning to apply once the window opens, foundational corporate and background check registrations must be completed before an application can even be initiated. Idaho does not require a Certificate of Need (CON) for DDAs.

4. Licensure and Certification Requirements

To provide Day Habilitation, an agency must be certified as a Developmental Disability Agency (DDA) under IDAPA 16.03.21. The certification process requires a comprehensive review of the agency's operational policies, staffing plans, and physical facility.

Once the application pause is lifted, applications are submitted via the IDHW Licensing and Certification portal. Facilities must also pass a physical inspection to ensure compliance with state and local safety codes.

5. Medicaid Provider Enrollment

After obtaining DDA certification, the agency must enroll as a billing provider with Idaho Medicaid. This is done electronically through the Idaho Health PAS OnLine portal, managed by Gainwell Technologies.

Providers must establish a Trading Partner Account to access the enrollment application. Federal regulations require all enrolled Medicaid providers to revalidate their enrollment at least every five years.

6. Staffing, Training and Background Checks

Idaho mandates strict qualifications for staff delivering Day Habilitation services. Program design and oversight must be conducted by highly qualified professionals, while direct care staff must meet baseline education, training, and background requirements.

No staff member may provide direct services or access participants until they have received an official clearance from the IDHW Background Check Unit.

7. Documentation, Policies and Records

DDAs are required to maintain extensive documentation to prove compliance with IDAPA 16.03.21. The foundation of this is the agency's Scope and Service Policy manual, which must be approved by IDHW before certification is granted.

Participant records must clearly link the daily activities provided in the Day Habilitation setting to the goals outlined in the participant's individualized plan of service.

8. Billing, Rates and Claims

Day Habilitation claims are submitted to the Idaho Medicaid Enterprise System (MES) via the Health PAS OnLine portal. Services are reimbursed based on a fee schedule established by the IDHW Division of Medicaid.

Providers must ensure that all billed services are prior-authorized in the participant's plan of service and that claims are filed within strict timely filing limits.

9. Approval Sequence and Timeline

The approval process for a new Day Habilitation provider in Idaho is strictly sequential, though currently halted by the state moratorium. An agency cannot apply for Medicaid enrollment until DDA certification is fully approved.

When the application window is open, the policy review and facility inspection phases are the most time-consuming, often taking several months to complete before Medicaid enrollment can begin.

10. Common Denials and Survey Findings

Applications for DDA certification are frequently delayed or denied due to incomplete policy manuals that fail to address every stipulation in IDAPA 16.03.21. Currently, any application submitted will be denied outright due to the moratorium.

During post-certification surveys, the most severe citations usually involve background check violations or failing to document that services provided matched the participant's plan of service.

11. Key Contacts and Resources

Prospective Day Habilitation providers must interact with multiple divisions within the Idaho Department of Health and Welfare, as well as the state's Medicaid fiscal agent.

Utilize the official state portals for the most current rule manuals, application updates, and moratorium announcements.
