Your Medicaid Billing Has an Order: The 3 Steps Home Care Providers Miss

New home care providers ask us the same thing: why can't I bill Medicaid yet? The answer is almost always the sequence — and the good news is the sequence is learnable.

Becoming a Medicaid provider isn't one application. It's three separate steps, each handled by a different office and each depending on the one before it. Skip a step or do them out of order, and you wait. Here is the order that actually gets a new home care provider paid.

Step 1: Get certified or approved by your state

Start with certification or approval through your state's program agency. This is the foundation, and it comes before Medicaid enrollment. Expect to show that your business is properly formed and that you have the policies and documentation your service line requires.

Step 2: Enroll as a Medicaid provider, with your NPI in hand

Once you're certified, you enroll as a Medicaid provider. Enrollment creates your billing relationship with the program, and it requires your National Provider Identifier. Until enrollment is complete, there is no active record to bill against, which is why providers who start here end up waiting.

Step 3: Contract with the MCOs that reimburse you

In many states, Medicaid services flow through Managed Care Organizations. After enrollment, you contract and credential with the MCOs in your area. Each one has its own paperwork and timeline, so track them separately. This is the step that finally opens the door to getting paid.
